Registration Fees

Physician $160.00 +HST

Other Health Care Professionals $120.00 +HST

Resident/Student $80.00 +HST

*Registrants will have the option to select in-person or virtual attendance during the registration process.

CONFIRMATION OF REGISTRATION

A written acknowledgement of your registration will be sent before the activity. Receipts are emailed at the time of registration, and an additional email will be sent after the activity, confirming your certificate of attendance is ready to download or print. Your registration is not complete unless a confirmation is received. If you have not received a confirmation within seven days of registration, please contact the CPD office.

DISCLOSURE OF POTENTIAL CONFLICTS OF INTEREST

Keeping with accreditation requirements and the National Standard for Support, McMaster University Continuing Professional Development  (CPD) requires all speakers, planning committee members, moderators, facilitators and authors participating in this activity, must disclose all relationships with for-profit and not-for-profit organizations over the previous two years. The disclosure must be done in print, verbally, and in writing on a slide before the speaker’s presentation.

LIABILITY

Continuing Professional Development Office (CPD) hereby assumes no liability for any claims, personal injury, or damage:

  • To any individual participating in this activity.

  • That may result from the use of technologies, programs, products and/or services during this activity.

  • That may arise out of, or during this activity.

PERSONAL SUPPORT  (In-person event)

If you require personal support at an in-person activity, the health aid provider must register in advance at the general public rate.

MEAL PACKAGES  (In-person event)

Meal packages for guests at an in-person event may be purchased. Contact the event coordinator for more information.

 

ACTIVITY PACKAGES/LANYARDS  (In-person event)

As the registrant, your in-person activity package includes nutritional breaks and lunch. Therefore, your activity lanyard must be worn throughout the day. If a guest accompanies you to the activity, their meals are at their own cost and will be billed directly.

DIETARY REQUIREMENTS  (In-person event)

For those with dietary needs for an in-person event, some accommodation may be available. Contact the event coordinator for details.

PHOTOGRAPHY (In-person event)

Candid photos will be taken at in-person activities. Your in-person registration implies your permission for these photos to be used for future promotional material. Individuals in photographs will not be identified. Please visit the registration desk during the activity with any concerns.

VIDEO RECORDINGS (Virtual event)

This event will be recorded, and participants will receive access to the video recordings after the event for six months. However, only participants who log into the live event are eligible to receive a certificate of attendance for CME credits.

CANCELLATION POLICY

The University reserves the right to cancel an activity due to insufficient registration or any circumstances that are beyond our control. Cancellations received before September 15, 2023, will be refunded less a 25% administrative fee to a maximum of $50.00. No refunds or requests to change from in-person to virtual (or vice versa) will be permitted after that date.
